Directory of Services and Facilities for Older People as part of the NSF for Older People Standard 8 on Active Aging. This initially involved researching and mapping all relevant services across the Borough. The Directory is designed to work as a printed document, a web page with a search engine and a database for health professionals. The design was informed by a parallel case management project, which looked at unmet health and social care need among elderly people who are high level users of Emergency services. We collaborated with Age Concern and Tottenham & Wood Green Pensioners’ Action Group and other groups of older people.
